NYSHIP Coverage & Eligibility
If you’re a New York State employee or retiree, you may have NYSHIP (Empire Plan) coverage. NYSHIP provides benefits for medically necessary therapeutic massage when related to pain, injury, or illness. Coverage and reimbursement vary by individual plan.
NYSHIP covers therapeutic medical massage for conditions involving pain, injury, or disease when medically necessary.

Massage therapy can be a qualified medical expense under many FSA and HSA plans, making it easier to support your health while using your benefits wisely.
What to Know:
• Check Your Plan: Not all FSA/HSA accounts automatically cover massage therapy. Confirm details

• Doctor’s Prescription: Most plans require a prescription stating massage therapy is medically
necessary.

Please complete and submit both forms prior to your appointment.
Please note: A Prescription is required prior to the initial visit. Our Medical Team may set up your initial evaluation or contact your Medical Provider.

Prescription Requirements
We require a Prescription for Medical Massage.
• Written diagnosis with Diagnosis codes
• Prescribed treatment listed specifically as Medical Massage (not Physical Therapy)
• Date the prescription was written (Prior to initial treatment)
• Recommended frequency and duration of treatment
• Prescribing provider’s name, phone number, address, and credentials
